It's unlikely that ESX 5.0 is the problem since we're unable to reproduce this issue on our local setup. We have tested the appliance on 4.1/5.0/5.1/5.5 and have no issues.
However, in our dev environment, we deploy only to the local disk. If your ESX has a local disk or has other arrays, you might want to try them instead to isolate the root cause.
We're planning to release in a few months a new version that is based on a newer version of SLES, which hopefully should work around this issue.
